I don't like the outfit she's wearing, it doesn't really fit her personality, but this picture was really just so I could get a coherent idea of what a Shiarreth looks like. The spots down her side are unique to every individual, like human fingerprints, so yes, they are natural, not tattoos or whatever. Cool ears.
